ICSI Process

At Lifespan Birth, we guide every patient step by step through the Intracytoplasmic Sperm Injection (ICSI) process. This advanced procedure is performed with precision in our modern laboratory, giving couples facing male infertility the best possible chance of fertilization and pregnancy.

Step-by-Step ICSI Process

The ICSI procedure is carefully coordinated within an IVF cycle. Here’s an overview of how it works:

1Egg Collection
  • Similar to conventional IVF, the woman undergoes ovarian stimulation to produce multiple eggs.
  • Once the eggs reach maturity, they are retrieved from the ovaries through a minor surgical procedure called transvaginal ultrasound-guided oocyte retrieval (TVOR).
2Sperm Collection
  • On the same day as egg retrieval, the male partner provides a semen sample.
  • Alternatively, sperm may be collected through techniques like testicular sperm extraction (TESE) or epididymal sperm aspiration (PESA) in cases of severe male factor infertility.
3Sperm Injection
  • In the lab, a single sperm is selected from the semen sample or obtained through specialized techniques.
  • Using a fine glass needle, the selected sperm is injected directly into the cytoplasm of each mature egg, bypassing the natural fertilization process.
4Embryo Culture
  • After sperm injection, the fertilized eggs (now embryos) are cultured in the lab for several days under controlled conditions.
  • Healthy embryos are then selected for transfer into the woman’s uterus, typically on day 3 or day 5 after fertilization.
5Success Factors
  • Embryo Quality: Higher-quality embryos are more likely to implant and develop into a successful pregnancy.
  • Endometrial Thickness: An endometrial lining optimally prepared (usually 8-14 mm thick) is ideal for implantation.
  • Age: The woman’s age at embryo freezing can influence success rates, with younger age at freezing generally associated with higher success rates.

  • Clinic Protocols: Different clinics may have varying success rates depending on their specific protocols and technologies.

6Advantages of FET
  • Flexibility: Allows for genetic testing of embryos and for scheduling the transfer when the recipient’s body is most receptive.
  • Higher Success Rates: Some studies suggest that FET cycles have slightly higher success rates than fresh transfers, as the body has more time to recover from ovarian stimulation.

Why Precision Matters

ICSI differs from conventional IVF because fertilization does not rely on many sperm competing with each other. Instead, a single strong sperm is selected and injected directly into the egg. This precision helps couples with severe male infertility, low sperm count, or previous IVF failure achieve fertilization that may not have been possible otherwise.

International Patient Support

For overseas patients, we coordinate the process to minimize stress and travel time. Initial consultations and tests can often be completed in your home country, while the most critical steps, egg retrieval, sperm injection, and embryo transfer, are performed during a short stay in Bangkok.

FAQs About the ICSI Process

How long does the ICSI process take?

A full ICSI cycle usually takes 3 weeks, including stimulation, retrieval, fertilization.

Is ICSI painful?

No. Egg retrieval is done under sedation and embryo transfer is a simple, painless procedure.

Can sperm be retrieved surgically for ICSI?

Yes. If sperm is not available in the semen, techniques like PESA or TESA can be used.

What happens to extra embryos?

Healthy embryos not transferred can be frozen for future use or tested with advanced techniques like NGS.
